As a family physician/geriatrician caring for patients near/at the end of life for more than ten years, I have been involved in clinic, hospital, and palliative care and hospice services within a southern California Health Maintenance Organization (HMO) providing service for nearly a half of million patients.
Success over the years have included the creation of palliation care models, addressing pain as a vital sign and improving the nursing home environment for patients not able/willing to die at home or the hospital.
