Aims: To compare MONICA questionnaire of cardiovascular diseases, cardiovascular drug therapies, and cardiovascular risk factors with regard to clinical records registered by family physicians.
Methods: Patients was selected by random sample of municipal census. Data related to cardiovascular diseases, cardiovascular drug therapies and cardiovascular risk factors were obtained by interview of patients (MONICA questionnaire) and review of clinical records (PAPPS criteria of Family Physicians Spanish Society). The kappa index was used to determine the concordance between MONICA questionnaire and clinical records (gold standard).
Results: The mean age was 65.3 (SD 11.3) years old. 61.3% were women. In cardiovascular diseases, the Kappa index was 0.864 for myocardial infarction, 0.859 for angor pectoris, and 0.820 for ictus. In drug therapy, the kappa index was 0.895 for serum lipids reducing agents, 0.930 for diabetes mellitus drugs, 0.911 for antihypertensive drugs, and 0.802 for aggregation inhibitors agents. Finally, in relation to cardiovascular risk factors the kappa index was 0.661 for hypercholesterolaemia, 0.772 for diabetes mellitus, and 0.815 for high blood pressure.
Conclusions: The concordance between MONICA questionnaire and clinical records is high in cardiovascular diseases and cardiovascular drug therapies. With regard to cardiovascular risk factors, the concordance was lower in hypercholesterolaemia and diabetes mellitus, except for high blood pressure.
