A SOFTWARE FOR SOCIAL SANITARY INTEGRATION

BACKGROUND: Health workers (family doctor, nurse, midwife, physiotherapist, specialist and paediatrician) with social workers (specialized or not) need to integrate their daily work. Only a common software which enters all their services can realize today a real professional integration.
METHODS: Since 1.11.01 we have a common software which registers social and sanitary services made directly at citizens' home or in territorial buildings. Since 1.04.02 we have a unique entry point for home care. This software, named As.Ter., is developed on Java platform and every intra-net point of ASL 11 is able to access to data.
RESULTS: We have 451 professionals: health and social workers. Each citizen who meet for any reason our NHS (except for hospitalization) is registered with a contact. So every professional can enter his service or project in this unique software. Therefore we have a unique "patient file" for each person with social and sanitary data. Moreover, every professional can see what the other professionals have done about services, projects, drugs, aids and so on.
CONCLUSION: In 2005 family doctors cured in home care with a program 3768 patients, among these 712 were cured with other professionals simultaneously. 756 patients had a health structured program: 56 % the lower, 28 % the medium and 16 % the highest level for complexity of assistance. 836 patients were cured simultaneously by a health and a social worker.
